  • Any article that brings to light the problem of bad breath is to be commended. But as Director of the National Breath Center, I’d like to fill in some details. To begin, bad breath is primarily the cause of the bacteria that live within the biofilm (coating) on the top of the tongue and under the gums. This biofilm contains bacteria that are years old as they can live without air, all the while causing the odors of bad breath. While food particles will ferment causing an odor, unless someone has large, numerous cavities, this is a minor factor. Oral causes do amount to 90-95% of the odors of bad breath but they come from the tongue and under the gums. While cleaning your mouth is something to be done thoroughly and regularly, the research does not contain any correlation between the amount of cleaning one does and bad breath. In fact, one would think that the worse the gum disease, the worse the bad breath. But research has related the severity of bad breath more to whether there is bleeding when a gum exam is done, not how high the gum disease numbers go. Certainly one who smokes will never have fresh breath, but drinking and eating acidic foods do not affect one’s breath in a directly negative way, only because they tend to dry the mouth. And while tonsils can cause an odor, unless they contain tonsil stones there is no reason to be alarmed. I have seen far too many people in the last 22+ years who have had their tonsils out with no change in their breath.
So, what should one do? Scraping the tongue is a must but one must be careful as the farther back someone goes, the larger the taste buds and the more chance there is to cut one off. I have seen numerous people in various stages of tongue scraper destruction by being too aggressive. If we are talking about chronic bad breath, absolutely stay away from milk. Milk does nothing but coat the tongue leaving a sour-like odor to the tongue and breath. And as a protein, serves as food for the bacteria in the biofilm. As healthy as it is said to be, keep milk out of your diet. I would also add flossing as an essential oral hygiene practice. Brushing alone cannot get between the teeth and is not enough to remove odors.
At the National Breath Center, we recommend brushing and flossing twice each day; using wet gauze to clean the tongue a few times each day; and a professional strength mouthwash if necessary on the gauze and to rinse with. However, even with all this, people with chronic severe bad breath will not get relief. Those are the people we see regularly. That is because the biofilm can only be removed by a dental professional. It is actually like a living organism and holds on for dear life to the spaces between the 9-10,000 taste buds we have on our tongue. The process of removing the biofilm is called Tongue Rejuvenation. It is painless, and may take a number of visits, but when complete, the tongue is clean and healthy with no bacteria to cause bad breath. Coupled with any treatment to help the gums stop bleeding, all bad breath odors should be gone for good.
